Recruitment and training source status

The 2027 process removes Core Training entry and confirms a fresh MSRA sitting, 40% MSRA / 60% interview weighting and two online stations of up to 10 minutes: portfolio and clinical/ethical. Detailed scoring anchors and internal clinical/ethical sequencing remain unpublished.

How to compare Emergency Medicine training programmes

There is no defensible national league table. Compare how a programme delivers ACCS capability, supervision and a sustainable working life across its actual hospital footprint.

Run-through progression and support

The 2027 application route is run-through ST1 without a Core Training entry option. Check the advertised programme, ARCP and exam progression requirements, and support when a trainee needs more time to develop capability.

Rotation footprint and commute

Map every likely hospital, not only the lead teaching centre. Consider night travel, parking, public transport, relocation, childcare and how frequently rotations move. A prestigious placement can still create an unsustainable weekly pattern.

ACCS placement design

Ask where Emergency Medicine, acute medicine, anaesthetics and ICM are delivered; how workplace assessments are coordinated; and whether trainees feel part of their parent specialty while away from the ED.

Clinical breadth

Compare paediatric departments, major trauma, tertiary specialties, DGH autonomy, urgent treatment centres, Same Day Emergency Care and pre-hospital links. Breadth often comes from a rotation network rather than a single large hospital.

On-call and senior support

Ask who is resident overnight, when consultants are present, escalation thresholds, resus supervision, rota intensity and how breaks and recovery are protected. Published hours do not show the distribution of nights and weekends.

Teaching, exams and simulation

Look for protected regional teaching, local induction, simulation, exam support, ultrasound governance and accessible supervisors. Ask current trainees how often teaching survives service pressure.

Research, QI and special interests

If relevant, ask about TERN sites, academic posts, local trials, PEM or PHEM access, ICM dual training and how trainees get supervised time. A list of projects is less useful than knowing whether junior trainees can participate meaningfully.

LTFT, leave and trainee voice

Ask how LTFT rotations and nights are organised, how leave is coordinated across ACCS specialties and whether trainee feedback produces visible changes. Speak to more than one trainee and distinguish a single rota problem from a recurring programme issue.
