Factual records

Keep a compact, truthful experience pack

ANRO does not ask for a scored CT1 portfolio upload. These records help supervisors verify completed work and help you recall detail. Every draft must be edited by the signer and used only when it is true.

Taster confirmation draft

Include organisation, department, dates and total sessions; pre-assessment, theatre, recovery, pain, obstetric or emergency exposure actually attended; named supervisor; and a short statement that the letter confirms attendance rather than competence.

Draft template · adapt to your own evidence
Please amend or remove anything inaccurate. This confirms that [name] attended [department] for [dates/sessions]. Their observed activities included [factual settings]. They discussed [learning topics] with the team. This letter confirms attendance and participation only and does not certify independent anaesthetic competence. Signed [name, role, GMC number, contact, date].

Replace the placeholders with accurate details. Any signatory must verify the statements.

QI or audit confirmation draft

Record the standard or problem, dates, your exact role, baseline measurement, intervention, remeasurement and where results were presented. Do not call data collection 'leading the project' unless that was your role.

Draft template · adapt to your own evidence
Please edit this draft so it is factually exact. [Name] contributed to [project] between [dates]. Their personal work was [tasks]. The team measured [baseline], introduced [change] and [remeasured / has not yet remeasured]. [Name] presented [what] to [audience] on [date]. Outcome: [result]. Signed [supervisor details].

Replace the placeholders with accurate details. Any signatory must verify the statements.

Teaching confirmation draft

State learners, dates, number of sessions, topic, design and delivery role, how feedback was gathered and what changed. One lecture is not a teaching programme.

Draft template · adapt to your own evidence
Please correct any inaccurate wording. [Name] [designed/coordinated/delivered] [programme/session] for [learner group] on [dates]. Their role was [specific contribution]. Feedback was collected by [method] and reviewed with [supervisor]; the main change made was [change]. Signed [details].

Replace the placeholders with accurate details. Any signatory must verify the statements.

Research contribution draft

Name the study or network, approvals where relevant, dates, contribution and current output status. Distinguish protocol development, recruitment, data work, analysis and authorship.

Draft template · adapt to your own evidence
Please use only if accurate. [Name] worked on [study/project] from [dates] as [role]. Their contribution comprised [specific tasks]. Governance/approval was [status]. The project is currently [stage]; outputs are [published/accepted/submitted/none yet]. This does not imply authorship beyond the stated output. Signed [details].

Replace the placeholders with accurate details. Any signatory must verify the statements.

Your one-page example index

Keep one line per potential answer: situation, date, your role, objective evidence, outcome, feedback, learning and the official domain it may support. Remove patient identifiers. This is a recall aid, not an upload requirement.
