Get signatures that actually confirm what happened

Give each supervisor the underlying record and ask them to correct any inaccurate statement. The current guide requires an index sheet and verifiable evidence; check signatures, required registration numbers and removal of patient identifiers before upload.

Surgical taster or elective confirmation

Use a letter only after attending; ask the consultant to check the days and actual learning.

Draft template · adapt to your own evidence

Operative verification

The official format is the eLogbook consolidation report, not a substitute testimonial. Provide the actual report to the consultant for checking and obtain the required signature on every page.

Follow the eLogbook evidence instructions

Presentation or publication proof

Use the current category-specific evidence: programme or acceptance, slides/poster and attendance proof; for eligible publications, the PubMed ID or Web of Science reference and article or qualifying acceptance. A supervisor letter does not replace the required documents.

Final upload audit

Index sheet; the permitted evidence for each selected claim; readable dates, signatures and registration numbers; verified English translation if needed; no patient-identifiable details. Keep an offline copy and report upload problems at least 48 hours before the closing deadline.
