Evidence into better care
Discuss research and QI without inflating either
The post determines the balance of research, audit, QI and implementation. A panel needs an accurate account of your contribution and a credible way to support evidence-based care in this service.
Match ambition to the post and infrastructure
An academic centre may expect grant, trial, supervision or university responsibilities; another post may need recruitment, evidence implementation and useful local evaluation. Read the job description before presenting one model as superior.
Explain your actual research contribution
Be precise about question, design, approvals, recruitment, analysis, writing, dissemination and limitations. Authorship position alone does not show scientific understanding or team behaviour.
Use QI for a measured local problem
Define the system, involve affected people, test change, use outcome and balancing measures and plan adoption. A completed audit can demonstrate assurance; do not relabel every audit as QI.
Connect evidence to implementation
Explain how guidelines, evidence, local variation and patient preference shape a change, and how you would detect inequitable uptake or unintended harm.
Create opportunity for the team
A consultant contribution may include mentorship, fair project access, protected time, methodological partnership and reliable governance, not only personal publications.

Apply the evidence to consultant practice
Distinguish audit, quality improvement, service evaluation and research while defending causal claims and governance.
Scope: UK-wide research ethics and professional duties; specific approvals and local policies vary.
Choose the method for the question
Clinical audit compares practice with an explicit standard; quality improvement tests and adapts changes to improve a defined system; service evaluation examines how a service is functioning; research seeks to generate generalisable knowledge using an appropriate design. Labels alone do not determine governance classification, and projects can share methods. Seek the appropriate local research governance or R&D advice rather than assuming that calling something 'QI' removes information-governance or ethics considerations.
A good consultant candidate can explain why the method was chosen, what the population was, how outcomes were defined, and how approvals and patient information were handled. Publication is not a guarantee of clinical benefit, just as an unpublished project can produce important local improvement.
What to establish
- Define the question, intervention or exposure, target population and meaningful outcome.
- Clarify registration, approvals, data handling and oversight for the actual project.
Avoid confusing association with impact
A project may coincide with falling waiting times because staffing improved or demand declined. When discussing outcomes, include baseline trends, denominator changes, selection effects and balancing measures. For small QI cycles, do not claim causality more strongly than the data permit. Time-series run charts and repeated measurements may show sustainability more convincingly than a single before-and-after percentage.
Describe what you personally did. 'Co-authored a systematic review' is less informative than explaining your protocol role, screening decisions, analysis, critical appraisal and how the findings were applied. Research integrity includes transparent authorship and limitations.
Build research capacity without compromising clinical delivery
An advertised research interest should be matched to realistic time, supervision, infrastructure, recruitment feasibility and funding. A consultant can contribute by improving trial identification, mentoring trainees, developing multicentre collaborations or implementing evidence into practice. Not every post is designed around grant-funded academic outputs.
Show how patients benefit: safer intervention, better evidence, broader participation or more effective pathways. Be explicit about what would need separate approvals and what could be done within ordinary clinical governance.
Apply the teaching
An apparent drop in biopsy complications
A service introduces a new checklist and reports that complications fell from 4% to 1%. However, the post-change period included fewer complex cases and shorter follow-up.
