Verification, not portfolio theatre
Build evidence that survives a follow-up question
Most local fellow adverts do not publish a national portfolio score. Keep a small private index of the achievements you actually need to prove for this vacancy. Share only requested material and remove patient identifiers.

Fictional completed index
Connect a claim to a record and a limitation
Keep an index for your own preparation; send only what the employer requests through its approved route. A useful entry lets you retrieve the evidence and explain exactly what it does and does not establish.
Teaching entry
Claim: delivered three undergraduate bedside sessions. Record: dated plans, attendance totals and anonymised feedback; registrar observed one session. Personal role: designed and delivered the sessions. Limit: feedback assessed usefulness, not a change in knowledge. Follow-up to prepare: what changed between the first and last session?
Improvement entry
Claim: contributed to a discharge-summary project. Record: agreed measurement sheet, baseline collection and minutes accepting a proposed checklist. Personal role: defined two data fields with the supervisor and collected the baseline. Limit: checklist not yet introduced and no remeasurement. Accurate description: baseline audit and planned change; not a completed improvement cycle.
Clinical experience entry
Claim: supervised acute medicine experience during a four-month post. Record: dates, job description and supervisor confirmation of actual responsibilities. Limit: no independent registrar take. Keep reflections de-identified; patient notes, screenshots and identifiable case logs are not suitable application exhibits.
Make confirmation easy to verify
Send a supervisor a short factual draft with dates and source records, asking them to correct it. Do not prefill praise or claim an outcome they did not observe. Reconcile the final wording with the CV and application. Keep a private index of file name, date, verifier and related criterion, rather than a folder of unexplained certificates.
