Local application and shortlisting
How do Clinical Fellow and Trust Grade applications work?
Individual organisations advertise a vacancy, essential and desirable criteria, duties and employment terms. They shortlist against the job, assess candidates using their chosen format, make an offer subject to checks and agree written terms. This is not Oriel specialty recruitment.
Choose an appropriate grade and inspect the entire job description, person specification, rota and site footprint.
Verify GMC registration, any specialty or experience threshold, right to work and sponsorship position where relevant.
Match each essential criterion to a short, truthful example in the application or supporting information.
Follow the interview invitation; research the team and practise original clinical and non-clinical prompts at the advertised grade.
If offered, complete required checks and review salary basis, contract status, supervision, on-call and induction in writing.
Eligibility
GMC status, qualifications, clinical experience and any immigration or timing conditions are stated in the live vacancy. A generic title cannot substitute for them.
Shortlisting
The person specification is the working blueprint. Desirables vary by role; do not treat research, courses or publications as universal scoring categories.
Interview
The panel may ask about motivation, CV, safety, teamwork or clinical judgement and may include another exercise. Format and marks are not nationally prescribed.
Offer
An interview recommendation does not replace employment checks, contract review, grade/rota agreement or induction planning.
Job-specific evidence
What evidence does a local fellow application need?
Build a small criterion-to-proof index, not a fabricated national portfolio. Show actual work, personal contribution, dates, supervision, results and learning; never use patient-identifiable material.
Clinical competence to grade
Relevant case mix, ward or acute responsibility, escalation and safe limits.
What to do: Describe one real decision and the supervision available rather than claiming unsupervised competence.
Teamwork and communication
A specific MDT, handover, patient or family example and its safety effect.
What to do: Make your actions visible and explain what changed.
Teaching and improvement
Delivered teaching, measured audit/QI or an honest in-progress contribution.
What to do: Separate your role from the team's and completed results from plans.
Eligibility and work history
Exact qualifications, registration, dates, references and right-to-work information.
What to do: Check current documents early and ask the employer about any ambiguity.
Interview process
What may a Clinical Fellow interview assess?
The employer assesses suitability for its post at the advertised grade. Reasonable preparation spans motivation, relevant clinical work, safe escalation, teamwork, communication, professional judgement, values and your own evidence; use the invitation as the actual specification.

- Teamwork, patient communication, values and speaking up
- Ownership of teaching, QI, audit, research or leadership actually claimed
- Understanding of the rota, supervision and working within competence

A locally recruited post, not one national competition
Clinical Fellow, Junior Clinical Fellow, Trust Grade, Trust Doctor and FY3 often describe locally employed doctors (LEDs). The title does not fix the grade, duties, training recognition, interview format or contract. Start with the vacancy's person specification and the employing organisation; check the rota and written terms before accepting.
No national interview station or portfolio tariff
An employer may use an application sift and panel interview, with clinical, values or presentation tasks if the particular vacancy requires them. Do not transfer IMT, CST or consultant station weightings into a Clinical Fellow application.
Grade is more important than the word 'fellow'
An FY3/SHO-equivalent ward post, registrar-equivalent senior fellow and research or teaching fellow can have very different clinical scope. Read expected independent decisions, supervision, out-of-hours duties and qualifications rather than the title alone.
Employment is not a training number
A locally employed post can offer valuable supervised learning and evidence, but it is not automatically a recognised training programme, ARCP or route to CCT. Ask how the post supports your own goals; check the relevant specialty recruitment rules separately.
2026 England contract changes are in progress
In England, permanent contracts are the default for LEDs covered by the 2026 deal, including those on pay points equivalent to the 2002 or 2016 doctor contracts, unless a stated fixed-term exception applies. Employers should offer repurposed 2016 terms while a separate national LED contract is developed for 2027. Confirm your vacancy's written terms and do not assume these England arrangements apply across the UK.
Job-specific assessment
What to practise for the panel
The employer's invitation is authoritative. Use these as plausible functions, not promised stations: why this job, clinical decisions at grade, patient safety, teamwork, teaching or QI, and fit with the organisation. Every focused guide below uses original prompts and points back to job-specific practice.
Motivation and role fit
Show why this department, duties, grade and team are a deliberate match. Avoid a generic 'I love the NHS' script.
Clinical judgement
Assess and treat the unstable patient within your competence; name senior help, monitoring, reassessment and handover. Do not improvise a registrar-level plan at SHO grade.
Teamwork, communication and escalation
Use a real MDT example; show what you said, to whom and what changed. For conflict or a deteriorating patient, safety and timely escalation precede a neat story arc.
Governance and values
Show immediate patient care, truthful documentation, speaking up and subsequent learning. A memorised policy acronym without action is weak.
Evidence, teaching and QI
Explain your actual contribution and limitations. Make relevance to this job explicit instead of reading out the CV.
Questions to ask the team
Ask about supervision, induction, rota, training access and the real clinical case mix. Do not leave salary and contract assumptions unresolved.
Focused preparation
Go deeper on the interview tasks that need practice
These focused guides break the broader interview into specific question types, decision structures and original practice scenarios.
Motivation and fit
Combine the advertised work, your actual readiness and what you hope to learn. The department needs a colleague who can do this job safely, not a rehearsed expression of enthusiasm.
Clinical judgement
The strongest first response to an unstable patient is safe assessment and help at the right time, not a long specialty differential or unsupported heroics.
Teamwork and communication
The panel needs to hear what you did, what others knew and how the patient or team was safer afterwards.
Governance and values
Explain what protects the patient now, then what needs reporting, candour, documentation and organisational learning.
CV, teaching and QI
Follow-up questions reveal the difference between a title on a CV and work you personally delivered.
Questions to ask
An interview is also your chance to understand who will support you, what the rota actually entails and what is written into the offer.
Free practice questions
Original questions to practise with your job pack
These are original prompts for local doctor posts, not recalled employer questions. Choose those appropriate to the advertised grade and specialty, then practise explaining your own decisions aloud.
Motivation and fit
- What does this post ask you to do that you have already done safely, and where would you need support?
- Why this hospital and clinical service rather than another post at the same grade?
- Which essential criterion is strongest in your application and which will the panel need more evidence for?
Safety and teamwork
- A patient on your ward is deteriorating during a busy on-call shift. What do you do first and whom do you involve?
- Tell us about a handover that changed what happened for a patient.
- A more senior colleague asks you to do something you are not competent to do alone. How do you respond?
Evidence and development
- What was your own contribution to the improvement project in your application, and what remained unfinished?
- How would you use the supervision and teaching available here over your first six months?
- What questions would you need answered about on-call and the contract before accepting?

Common questions
Clinical Fellow & Trust Grade recruitment FAQs
Is Clinical Fellow a training grade?
Usually this is a locally employed post rather than a national training number. Confirm the contract, training recognition and supervision in the vacancy.
What is the difference between JCF and SCF?
Junior and senior labels suggest different expected responsibility, but are not standard grades. Read the person specification, rota and decision-making scope.
Is FY3 an official training year?
FY3 is commonly used informally for work after FY2. It does not create a third Foundation Programme year or ARCP.
Where should I find vacancies?
Start with NHS Jobs and employer career pages; search Clinical Fellow, Trust Doctor, Trust Grade and the actual specialty and grade. Review attachments.
Is there a national application calendar?
No single LED recruitment calendar. Jobs are advertised locally throughout the year, with employer-specific closing dates and interviews.
What does shortlisting score?
The employer's essential and desirable person-specification criteria. No national Clinical Fellow self-assessment tariff applies to every vacancy.
Do I need GMC registration?
Check the post's exact eligibility and your GMC registration/licence status; a clinical doctor role requires the appropriate UK professional authorisation.
Can an IMG apply?
Yes where the advertised eligibility is met. Check GMC registration, English-language evidence, immigration status or sponsorship and any NHS-experience requirement for that specific post.
Will the Trust sponsor a visa?
Do not assume so. Check the vacancy and recruiter; a Health and Care Worker visa depends on an eligible offered job, approved sponsor and current government conditions.
Do I need a taster or attachment?
Only if the specific person specification requires it. Observing the service can help your decision; it is not proof of independent clinical competence.
Does teaching or QI have a national score?
No universal LED portfolio score. Present real, verifiable contribution only when relevant to the published criteria and interview.
Does a publication compensate for missing essential clinical experience?
Usually no. Essential eligibility must be met; research may help distinguish candidates only where relevant to the post.
Can I use the same statement for every job?
Keep an evidence bank, but tailor the submitted statement to each organisation, level, specialty and person specification.
Will I be asked clinical questions?
Possibly, especially in a clinical rota role, but there is no national station format. Prepare safe clinical reasoning at the advertised grade and follow the interview invitation.
What if I am asked something beyond my competence?
Explain immediate safety steps, available information, what you can do, timely senior help, reassessment and handover. Do not claim independent expertise you lack.
How much Trust research is useful?
Know the sites, patient group, service, job responsibilities and relevant organisational priorities. One specific connection to the role is better than a memorised values list.
Do I need a formal portfolio at interview?
Follow the invitation. Keep evidence organised and factual, but do not arrive with confidential patient material or assume the panel requires a nationally prescribed binder.
Are on-calls part of every fellowship?
No. Check the actual rota, cross-site cover, senior availability, nights and weekends before applying or accepting.
Will I have protected teaching or study leave?
Terms vary by employer and contract. Ask what is written, what time is scheduled and what local budgets and supervision are available.
Does a fellowship count towards CCT?
It does not automatically count as a national training post. Evidence may be relevant to later applications or a separate approved route; check specialty-specific rules.
Can a fellowship support CREST?
It may provide supervised opportunities to demonstrate competencies, but sign-off must meet the current form and recruitment conditions. A job title alone is insufficient.
How does appraisal work outside training?
Ask the employing organisation about appraisal, designated body and responsible officer. You remain responsible for GMC revalidation evidence across your scope of practice.
Is a fixed-term contract always normal?
Do not assume it. England's September 2026 guidance expects substantive LED contracts by default unless a legitimate fixed-term reason applies; get the precise local terms in writing.
Does an FY3 JCF qualify for the SAS transition framework?
Not by title. The England framework has conditions including at least 24 continuous months with the same employer in SAS-comparable duties and other experience requirements.
Can I work LTFT?
Ask the employer about the specific post, rota and contractual arrangement. Locally employed posts are not governed by one national training LTFT allocation process.
Should I pay for an interview course?
Not by default. First use the job pack, original practice prompts, feedback from a colleague and a mock aloud; buy a course only if its added value addresses a real gap.
Can I use specialty-training guides to prepare?
Yes for evergreen clinical and specialty understanding, but do not import national training station formats or scoring into a locally recruited fellow post.
What should I ask before accepting?
Confirm rota and sites, on-call senior cover, grade and pay basis, induction, named supervisor, development time, leave, contract length or permanence and appraisal.
