Application and scoring
How Paediatrics ST1 selection works
Applications close at 4pm UK time on 19 November 2026. Interview invitations and the declaration are sent on 20 January 2027; booking and declaration return close at 4pm on 22 January. Interviews run 2 February–25 March. The published 2027–28 guides confirm the application and scoring below.
Meet the 2027 person specification, including recent Foundation competence and no more than 24 months whole-time-equivalent Paediatrics experience outside Foundation modules.
Submit one national Oriel application and complete every eligibility, employment and supporting-form requirement accurately.
Use concise written evidence across the five shortlisting areas; two assessors independently score the form.
If invited, complete the cycle-specific interview across Communication, Career Motivation, Reflective Practice and Paediatric Clinical Reasoning.
Meet appointability, receive a national rank and preference only the programmes you would genuinely accept.
Written application
Each of two assessors scores the form out of 30 across five areas. These marks are not added to the interview score; the applicant guide includes the shortlist score among tie-break criteria.
Interview
Four domains each carry 40 marks across two independent assessors. Interview performance determines national rank, with tie-break rules in the applicant guide and prioritisation applied to offers.
Appointability threshold
The 55% appointability threshold does not guarantee an offer. Serious safety or probity concerns can still make a candidate unappointable.
Portfolio and evidence
What evidence should you build?
The current rubric rewards distinct examples, personal action and reflection. Shortlisting answers use 50-word text boxes without supporting evidence uploads. Keep records for accuracy; eligibility and adjustment documents have separate requirements.
Transferable clinical capabilities · 0–8
Up to five distinctly different capabilities, no more than two practical procedures, each made relevant to Paediatrics with reflection.
What to do: Balance communication, handover, teamwork, leadership, organisation and safe clinical work rather than repeating one strength.
Personal achievements and reflection · 0–8
Up to five medical or non-medical achievements, separate from the clinical examples, with impact and development explained.
What to do: Choose for breadth and specify what you did, who benefited and what changed in your later practice.
QI or audit · 0–5
Only the best of up to two projects is marked. Explain planning, presentation and results or recommendations rather than relying on a grand title.
What to do: Finish a bounded local project, present it and record the result or recommendation before chasing a national venue.
Academic work · 0–3 plus 1
Describe the actual research contribution and output; a listed research qualification can add one point but does not replace project evidence.
What to do: Agree a defined role, supervisor, output status and authorship expectations at the start.
Teaching · 0–5
The scale moves from limited delivery to design, leadership, evaluation and using feedback to improve later sessions.
What to do: Repeat one useful session after a documented change; a small completed programme is defensible evidence.
Interview process
What does the 2027 interview assess?
The 2027–28 applicant guide confirms two 20-minute online stations and four domains of up to ten minutes. Communication has five minutes of reading before interview. Follow your booked slot and joining instructions for overall attendance.

Shortlisting builder
Translate the five published 2027–28 descriptors into distinct, specific and reflective written evidence.
Evidence pack
Keep factual records that supervisors can correct and that make application and interview claims easier to defend.
Avoid portfolio theatre
Put effort into finishable evidence that meets a published descriptor instead of certificates that merely sound relevant.
Getting involved
Use supervised exposure and small governed contributions to learn how child-health services actually work.
Working life
Compare the pace, decisions and coordination across settings instead of treating one ward timetable as the whole career.
On-call
See what comes through, who calls, how work is prioritised and when an early trainee should escalate.
Training & Progress+
Follow capability development through two curriculum levels without inventing one rigid national sequence of rotations.
MRCPCH exams
See what each examination is for and keep exam progression connected to workplace capability.

2027 Round 1 · checked 30 October 2026
Before you submit: deadline, competence and priority status
Complete these national checks alongside the specialty's eligibility, evidence and selection requirements.
Application deadline and limit
Round 1 applications close at 4pm UK time on 19 November 2026. The national limit is five applications per recruitment round. Some specialties also require you to choose a single regional vacancy, so check the application route before submitting.
Foundation competence and CREST
If CREST is required, national guidance accepts valid 2024 or 2027 forms for 2027 recruitment. Required competence evidence must accompany the application; there is no later upload opportunity. Check validity against the specialty person specification and resolve any conflicting specialty instructions before relying on an older form.
Check your priority status in Oriel
For standard 2027 specialty recruitment, prioritisation applies to interview invitations and training offers. Check the status displayed in Oriel after applications close. Round 1 requests to review an incorrect status are due by 5pm UK time on 24 November 2026. Priority status does not replace eligibility or appointability requirements.

The published Paediatrics ST1 requirements for 2027
RCPCH has published the 2027–28 applicant guide and shortlisting rubric for August/September 2027 entry. They confirm the written application, two online interview stations and scoring described below.
The 2027 person specification is current
Applicants need a recognised medical degree, full GMC registration when applying, a licence at post start and recent Foundation competence through an accepted route. The specialty criteria include safe initial management of an acutely ill young patient, working with children and carers, coping with pressure, safeguarding awareness and informed commitment.
The paediatric experience ceiling is 24 months
The 2027 person specification permits no more than 24 months whole-time-equivalent experience in Paediatrics at application, excluding Foundation modules. All non-Foundation paediatric posts count, wherever they were undertaken. There is no minimum Paediatrics experience for ST1 entry.
2027 dates and virtual interviews are confirmed
For August/September 2027 entry, applications close at 4pm UK time on 19 November 2026. Invitations and the declaration are sent on 20 January; booking and declaration return close at 4pm on 22 January. Interviews run 2 February–25 March 2027. RCPCH lists confirmed regional dates and may add further capacity.
The 2027–28 applicant and scoring guides are available
Use the current applicant guide for interview rules and the separate shortlisting glossary for the five application domains. They apply to posts starting in August or September 2027; follow your Oriel invitation for booking and joining instructions.
One UK application and one transferable score
A single Oriel application covers participating regions across England, Scotland, Wales and Northern Ireland. Eligibility is checked before written applications are shortlisted. Interview performance determines national rank; offers also depend on preferences and the applicable prioritisation rules.
Recruitment prioritisation applies in 2027
The 2027 person specification states that the Medical Training (Prioritisation) Act 2026 applies. Priority status and assessment performance are separate parts of recruitment; read the national rules and verify the status shown in Oriel.
Competition figures need a year label
RCPCH reports 2,605 applications for 508 ST1 posts in 2026–27, a competition ratio of 5.13 and a final fill rate of 100%. These completed-round figures cannot predict the 2027 shortlist score or offer rank.
Official 2027–28 interview
Map each interview domain to the function it tests
The 2027–28 guide specifies two 20-minute online stations. Four domains each last up to ten minutes and carry 40 marks across two assessors. Follow your booked slot and joining instructions for overall attendance.
Communication · explanation with a role player
Applicants receive a scenario five minutes before interview and explain a clinical condition, intervention or transfer to a patient, parent or carer. The task assesses content plus the quality of interaction. Prepare to acknowledge emotion, involve the young person appropriately and check understanding.
Career Motivation · a virtual selected portfolio
Applicants do not show a physical portfolio. They use selected achievements and experience to demonstrate enthusiasm, suitability, understanding of the specialty and the attributes they would bring. Assessors do not see the application form.
Reflective Practice · change after a significant event
The official task asks applicants to reflect on a clinical event that went well or badly, show how it modified practice and explain how learning was shared. The guide advises spending little time retelling the event.
Paediatric Clinical Reasoning · safe F2-level thinking
The scenario tests a clear, methodical approach, relevant issues, management, limits and escalation. It is designed so applicants without a prior paediatric post are not disadvantaged and does not require complex specialist knowledge.
Appointability and serious concerns
The threshold is 88/160. A candidate can still be vetoed for serious patient-safety or probity concerns; a station score of 25% or below from either assessor triggers review with the Clinical Lead. Safe escalation and honest limits therefore belong in the answer, not as an afterthought.
Paper notes are allowed; keep them brief
The 2027–28 guide allows applicant-prepared paper notes and warns that reading them could disrupt engagement. Use a small cue sheet for names or structure, maintain eye contact and answer the question actually asked.
Focused preparation
Go deeper on the interview tasks that need practice
These focused guides break the broader interview into specific question types, decision structures and original practice scenarios.
Paediatrics communication
Paediatric communication is more than simplifying medical language. Strong answers acknowledge emotion, involve the child appropriately and make risk and next steps understandable to families.
Career Motivation
Select a small number of experiences that prove why the work suits you, what you understand about the difficult parts and which attributes you would bring to training.
Reflective Practice
The event is only the starting point. The assessed work is how you analysed it, what changed in your practice and how the learning reached other people.
Clinical Reasoning
The entry-level task rewards a clear safe method and recognition of limits. Specialist recall is less useful than identifying the sick child, acting, reassessing and escalating.
Free practice questions
Original Paediatrics ST1 practice questions
These prompts are written from the published domain functions and 2027 person specification. They are not recalled interview questions.
Communication and Career Motivation
- Explain to a worried parent why their child needs transfer to a specialist centre.
- A young person wants part of the history kept confidential from a parent. How would you approach the conversation?
- Why Paediatrics, and which experience most changed your understanding of its difficult parts?
- Which three experiences would you select to demonstrate your suitability, and what does each add?
Clinical Reasoning and Reflective Practice
- A child with breathing difficulty is worsening after initial treatment. Talk through assessment, action, reassessment and escalation.
- A parent says their child is getting worse although the observations are unchanged. What would you do?
- Reflect on a clinical event that changed how you work and explain how you shared the learning.
- Describe something that went well because of the team. What did you learn beyond congratulating the group?

Common questions
Paediatrics ST1 recruitment FAQs
What is confirmed for Paediatrics ST1 2027 recruitment?
The 2027–28 applicant guide and shortlisting rubric are published. They confirm the application scoring and two online interview stations. Invitations and the declaration are sent on 20 January, booking and declaration are due by 4pm UK time on 22 January, and interviews run 2 February–25 March 2027.
Do I need previous Paediatrics experience?
No. The 2027 person specification states there is no minimum Paediatrics experience for ST1 entry. You still need informed specialty understanding and evidence of transferable capabilities.
What is the maximum previous Paediatrics experience?
No more than 24 months whole-time-equivalent in Paediatrics at application, excluding Foundation modules. All non-Foundation paediatric posts count, including work outside the UK.
How is shortlisting scored for 2027?
Two assessors each score the written form out of 30: clinical capabilities 8, achievements and reflection 8, QI or audit 5, academic work 3 plus 1 for a listed research qualification, and teaching 5.
Do I upload evidence files for shortlisting?
No supporting files are uploaded for the shortlisting answers: assessors score the written descriptions. Keep accurate records. Documents for eligibility and reasonable adjustments may still be required separately.
How long can the application answers be?
The 2027–28 rubric uses 50-word text boxes and some dropdowns. Each box has its own limit: make your action, relevance and reflection explicit rather than treating 50 words as one allowance for an entire domain.
Does every QI project need multiple cycles?
The current descriptor focuses on planning or coordination, presentation and results or recommendations. Multiple cycles are a dropdown description but not the only route to a credible example. Represent the project stage accurately.
Does my QI, teaching or research need to be paediatric?
The 2027–28 guidance focuses on relevance to application level and experience rather than requiring every project to be paediatric. Explain the transferable learning honestly and check the exact current descriptor.
Should I sit MRCPCH before applying?
MRCPCH is essential during training but is not required for ST1 entry. The current rubric has no MRCPCH points category, so an early exam does not itself create shortlisting points.
Is a paediatric life-support course required?
No such course appears as an ST1 eligibility requirement in the 2027 person specification. Attend when it supports your role or learning, not because a commercial provider implies it is mandatory for application.
How can I arrange a useful taster?
Ask the Foundation team, paediatric clinical tutor or department with specific objectives across acute assessment, ward work, clinic, neonatology or community care. Record what you learned, not only attendance.
What is the interview format for posts starting in 2027?
Two online 20-minute multi-scenario stations cover four domains of up to ten minutes each: Communication, Career Motivation, Reflective Practice and Paediatric Clinical Reasoning.
How many interview marks are available?
The 2027–28 guide allocates 40 marks to each of four domains, totalling 160 from two independent assessors in each station.
What is the appointability threshold?
It is 88/160, equivalent to 55%. This does not guarantee an offer: rank, preferences and prioritisation still apply, and serious concerns can make an applicant unappointable.
Do interviewers see my application form?
No. Assessors do not have the application form during interview. Prepare selected experiences for the Career Motivation discussion; you are not required to display a portfolio.
Can I use notes in the interview?
The 2027–28 guide permits applicant-prepared paper notes. Electronic notes and books are not allowed. Use brief cues and respond to the actual question; reading a prepared speech can disrupt interaction.
How specialist is the Clinical Reasoning domain?
The 2027–28 guide sets it at UK F2-equivalent level; applicants without a paediatric post should not be disadvantaged. Show method, safe management, limits, reassessment and escalation.
What makes a good reflective example?
Use a significant event you can discuss without breaching confidentiality, spend little time on the story and show analysis, learning, change in practice and appropriate sharing of learning.
How should I prepare for the Communication domain?
Practise explaining a condition, intervention or transfer to a child, young person or carer. Acknowledge emotion, adapt to development, chunk information, check understanding and make safety-netting clear.
What is Progress+?
It is the RCPCH curriculum with 11 domains and two training levels: Core across ST1–ST4 and Specialty across ST5–ST7. Progression is capability based rather than a rigid national rotation sequence.
How long is Paediatrics training?
The usual indicative pathway is seven years from ST1 to ST7, with Core then Specialty level. Calendar length can vary through capability progression, LTFT, leave or approved out-of-programme activity.
What examinations make up MRCPCH?
Foundation of Practice, Theory and Science, Applied Knowledge in Practice and MRCPCH Clinical. The three theory exams must be passed before the clinical examination.
What is the difference between subspecialty training and a SPIN?
Subspecialty training leads to a CCT in one of the approved paediatric subspecialty programmes. A SPIN adds focused expertise to General Paediatrics and is not itself a GMC-recognised subspecialty CCT.
How much neonatal work is involved?
Neonatal capability is part of Core training, but the amount, unit level and out-of-hours responsibility vary by programme. Ask where neonatal placements occur and what early trainees do at deliveries and on call.
What does an ST1 do on call?
Usually supervised assessment and initial management of acute referrals and ward patients, with neonatal duties varying locally. The registrar and consultant should be used early for deterioration, uncertainty, safeguarding or workload beyond safe capacity.
Is Paediatrics mainly communicating with parents?
No. Paediatricians communicate directly with children and young people according to age and development while working with parents or carers. Autonomy, confidentiality, assent, capacity and safeguarding all matter.
Can I train less than full time?
Yes. LTFT changes the schedule, while the same curriculum capabilities remain. Check the programme's current application process, rotation design and on-call arrangements.
How should I compare regions?
Compare geography, DGH and tertiary balance, acute, neonatal and community exposure, step-up support, teaching, MRCPCH help, LTFT, research, rota design and commute. Do not use unsupported league tables.
What current issues should I understand?
Start with State of Child Health 2026, vaccination and prevention, inequalities, mental-health and neurodevelopmental demand, workforce gaps, transition and safety learning. Connect each issue to a clinical consequence and realistic action.
Are candidate-reported cutoff scores reliable?
No. Public reports may omit prioritisation, ties, capacity and later changes. Use the official cycle documents and Oriel feedback; treat online cutoff discussion only as unverified context.
Where should I practise next?
Start from the four focused guides linked from this hub, use their original questions and rehearse aloud. The Paediatrics product page explains the current availability of Example Bank, Coach and Interview practice.
